Please note the name(s) must be in the order and as they appear on your child’s birth certificate. We cannot accept any shortened forms or alias, this is to ensure the most accurate registration possible.
Please be aware that once this form has been submitted, it will be held within your child’s health record.
You can use this service if you:
- want to register at the surgery
- live within our catchment area
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
- details of previous GP surgery
- basic health and medical information
