- Please note your name(s) must be in the order and as they appear on your birth certificate and or any official photographic identification you may have. We cannot accept any shortened forms or alias, this is to ensure the most accurate registration possible. Please enter your NHS number (if you have one) to aid in this process – you should have one if you have ever seen a GP in the UK
- Please only submit this form when you are due to begin studying with Wymondham College. If you submit this form early we will register you with us while you may not actually be at college
- Once this form has been submitted, it will be held within your health record

- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
- details of previous GP surgery
