When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Once you have submitted your online form, you will need to provide the surgery with ID and a signed letter asking to be registered with the medical centre.

As we are currently advising patients not to attend the practice unless strictly necessary we can only accept copies by email or post.

Please note until you have provided this we will not be able to process your registration. In some circumstances we may be able to accept other types of ID which will be reviewed on a case by case basis.

Please note your name(s) must be in the order and as they appear on your birth certificate and or any official photographic identification you may have. We cannot accept any shortened forms or alias, this is to ensure the most accurate registration possible. Please enter your NHS number (if you have one) to aid in this process – you should have one if you have ever seen a GP in the UK.
